Enhancing Access2Care is a two-year program that expands access and improves care for underserved populations throughout the United States. The program, funded by Teva Pharmaceuticals and implemented together with Direct Relief and Volunteers in Medicine national office, provides funding over a two-year period to Volunteers in Medicine clinics to support the delivery of primary and preventive health care for patients with multiple chronic conditions (MCC).
In the first year of the Enhancing Access2Care program, four grantees supported 421 patients with MCC, including diabetes, pre-diabetes, hypertension, and hypercholesterolemia / hyperlipidemia. Each program represents an innovative, scalable approach to meet the needs of vulnerable patients with MCC.
